Thank you for inquiring about our services. Below are client and patient forms and fee schedules. Helping your pet is the most important thing to us. However, addressing financial questions early on and as completely as possible can help prevent any misunderstanding later, and make it easier for you to plan for your pet’s therapy.
1) If you make any appointment with us, please fill out the patient / client form and return it by fax (707-745-9689) or drop it off at the office. You may bring it on the day of your visit, however getting it to us in advance allows us to prepare your pet’s record ahead of time.
2) If you make an appointment with us for acupuncture, herbal therapy, spinal manipulation or laser phototherapy, please fill out the following additional forms and fax them to us (707-745-9689) or drop them off at the office ahead of time. You may bring them on the day of your visit, but getting them to us in advance allows us to prepare your pet’s record and familiarize ourselves with your pet’s condition. You may also email them to us – call the office for our email address (707-745-1135).
3) If your pet has had X-rays taken, please bring them with you since they can provide valuable insight into your pet’s problems, especially if we will be doing acupuncture for joint or back problems. You will need to sign them out from your veterinarian. We will return them to you and you will need to bring them back to your veterinarian. Although it is not essential, a copy of your pet’s medical records can be very useful, especially any blood work or other diagnostic test results.
